Licensed or Registered Dietitian/Nutritionist - Home Health Visits PRN/Per Diem | Port St Lucie, FL Pay: $60 per visit Trilogy Home Healthcare, a Humana company, is hiring a dedicated and compassionate Licensed or Registered Dietitian/Nutritionist to join our award-winning team. What You’ll Do: Provide dietary and nutritional counseling to home health patients Develop and implement nutrition care plans based on physician orders Collaborate with interdisciplinary clinical teams to support patient outcomes Educate patients and families on nutrition management and healthy lifestyle choices Complete timely and accurate documentation Why Trilogy?
