MDS Coordinator

Butler Rehab & Healthcare located 416 S High street, Butler, MO is seeking an MDS/Care Plan Coordinator to join our friendly team. Job Type: Full-time MDS Coordinator - Benefits: Competitive Pay. Comprehensive Benefits Package. Hands on upper management. PTO. MDS Coordinator- Requirements: High school diploma or equivalent. MDS Experience: 1 year (Preferred). RN/ LPN License. Responsibilities of MDS/Care Plan Coordinator: Actively participate in the development of a comprehensive, individualized plan of care for each resident. Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care/needs. Assume responsibility for implementation of Clinical and Prospective Payment System Minimum Data Set completions schedule Direct, organize and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care and Health Maintenance Organization (HMO) reimbursement Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements Participate in resident level of care determination upon admission Review reports of timely completion of residents\' MDS assessments and care plans and take follow up action as necessary Analyze Quality indicator reports for potential resident concerns and problems and review residents\' care plans to be sure this information is included Will maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning and the prospective payment system (PPS).
