Position Summary

The Licensed Practical Nurse (LPN) provides direct patient care and clinical support to the physician and care team in a concierge primary care practice. This role plays a vital part in delivering a personalized, high-touch patient experience by assisting with clinical care, coordinating follow-up services, supporting preventive health initiatives, and fostering strong patient relationships.

The LPN serves as a trusted resource for patients, helping ensure continuity of care, exceptional service, and adherence to clinical standards and practice protocols.

Key Responsibilities

• Build meaningful relationships with a select patient panel.
• Work closely with our physician in a true collaborative partnership during patient examinations and procedures, ensuring a comfortable and efficient environment.
• Implementation and management of Chronic Care Management (CCM), including accurate documentation, patient enrollment, monthly call compliance, and revenue contribution
• Provide direct patient care including vital signs, medication administration, injections, immunizations, wound care, and diagnostic testing within scope of practice.
• Manage patient records?by accurately inputting information into electronic health records (EHR) and maintaining confidentiality in accordance with HIPAA regulations. 
• Coordinate comprehensive care plans and follow up with patients personally.
• Serve as a primary point of contact, responding to patient needs with care and attention.
• Apply expanded clinical skills in a thoughtful, patient-centered environment.

Qualifications

Required Qualifications

• Graduate of an accredited Practical Nursing program. 
• Current Licensed Practical Nurse (LPN) license in the state of practice. 
• Current American Heart Association BLS Certification. 
• Phlebotomy experience or demonstrated proficiency in blood collection. 
• Minimum two years of clinical nursing experience, preferably in primary care, family medicine, concierge medicine, or an outpatient setting. 
• Strong clinical, communication, and patient service skills.

Preferred Qualifications

• Experience in a clinical setting, particularly in outpatient practices.
• Strong clinical assessment and triage skills.
• Familiarity with specific EMR systems (e.g., EPIC, Athena).
• Experience with patient education and health promotion.
