DayHab Brooklyn New York - Senior Rehab Technician 25 Elm Pl, Brooklyn, NY 11201, USA Job Description Reports to: Habilitation Specialist, Supervisor General Purpose: A Direct Support Professional (DSP) supports people who have intellectual/developmental disabilities and/or autism spectrum disorder to increase their independence and daily living skills. The Senior DSP assists the Habilitation Specialist or Supervisor in managing daily operations, meeting program goals, and overseeing programmatic quality. Essential Functions Ensure person‑centered surveys are completed monthly, compile results, and share them with the team for program planning. Develop site and staff schedules with the Supervisor based on participants’ interests, goals, and support needs. Ensure all team members follow program schedules, maintain attendance records, and make any necessary schedule changes promptly. Notify the Supervisor and follow the chain of command when team members are not receptive to assignments or discussions. Act as point person for community site groups; keep the Supervisor informed of status and report issues immediately. Ensure team members document services, time, and attendance in the EHR by COB, according to Medicaid and DQA regulations, and verify accuracy of service units. Correct any QA‑identified discrepancies in real time within the given timeframe. Collaborate with the Supervisor to ensure services are delivered as designed in Staff Action Plans and maintain seamless provision throughout the day. When the Supervisor is not onsite, maintain chain of command for daily operations and emergency response. Identify and submit person‑centered monthly vocational and activity goals with the Community Development Specialist, and attend monthly goals meetings. Maintain regular contact with volunteer sites, update the volunteer site list as needed, and ensure volunteer hours are recorded in the EHR. Collect and record performance data from internships, volunteer, and community sites. Report incidents as mandated by OPWDD and NYS regulations. Maintain updated Monthly Fire Drill logs at each hub site. Order and inventory supplies for each site, coordinated with the Office Manager, using D365 for requisitions, check requests, petty cash, and reimbursements. Manage metro card distribution, logs, and reconciliation; ensure sufficient cards are onsite to support at least two weeks of community scheduling. Maintain balanced petty cash, reconcile receipts monthly, and ensure funds are available for two weeks of scheduled activities. Stay within the program budget for activities and supply orders, requesting approval for purchases that exceed budget. Scope of Responsibility & Positions Supervised Ensure that all team members follow the program schedule and allocate staff coverage as required. Ensure that volunteer and internship hours are documented daily in the EHR. Ensure that services and unit attendance are documented daily, and that QA corrections are made contemporaneously. Provide daily operations training to DSPs and address performance issues, escalating to the Supervisor as needed. Maintain program petty cash and metro cards. Supervised by a Habilitation Specialist or Supervisor. Special Working Conditions Daily travel on public transportation is required. DSP location assignments may change with advance notice based on coverage needs. Qualifications / Basic Job Requirements HS Diploma, GED, or suitable equivalent. 6+ months of human service experience, or life experience with persons with intellectual or developmental disabilities (preferred).
