Responsibilities Perform physical therapy evaluations and treatments for a mix of inpatient and outpatient cases. Support mobility progression from acute care through SNF, including discharge planning and coordination with care teams. Provide therapy in ER, ICU, Med/Surg (Acute, Observation, and SNF), with occasional work in OB and extensive activity in the hospital-based outpatient clinic. Maintain a full schedule with 30–45 minute outpatient visits and 1-hour new evaluations; typically 5–15 patients per day. Document treatment plans, progress notes, and discharge summaries; communicate with physicians, nurses, and case managers to optimize patient outcomes. Demonstrate independence in a rural Kauai setting, adapting approaches to diverse outpatient diagnoses and ensuring continuity of care.

Requirements Hawaii Physical Therapy license required (ability to obtain promptly) Ability to work across inpatient and outpatient settings (ER, ICU, Med/Surg, SNF) and hospital-based outpatient clinic Availability to work Saturdays as required by the assignment Experience with inpatient mobility progression and discharge planning is preferred Strong clinical judgment, autonomy, and commitment to patient-centered care
