RN Navigator

Kaiser Permanente seeks an RN Navigator to guide patients through the healthcare continuum, coordinating care, education, and resources. In this Healthcare & Medical Services role, you will assess patient needs, create individualized care plans, and collaborate with multidisciplinary teams to improve outcomes and satisfaction. You will provide evidence-based education, support care transitions, and track clinical and utilization metrics. The RN Navigator advocates for patients and families, optimizes use of Kaiser Permanente services, and supports quality, safety, and process-improvement initiatives across the care delivery system.
