We partner closely with physicians, caregivers, and interdisciplinary team members to restore function, promote independence, and improve quality of life. We are seeking a licensed Physical Therapist who is clinically skilled, documentation-savvy, and passionate about home-based care. Position Summary The Physical Therapist provides skilled physical therapy services to homebound patients in accordance with the physician’s plan of care, applicable state and federal regulations, and agency policies. Services focus on restoring function, improving mobility, reducing pain, and preventing disability. Essential Duties & Responsibilities The Physical Therapist will: Perform comprehensive physical therapy evaluations in the home setting Develop, implement, and update individualized Plans of Care (POC) in collaboration with the physician Provide skilled interventions including: Therapeutic exercise Gait and balance training Transfer and mobility training Pain management techniques Fall-prevention strategies Evaluate patient response to treatment and modify treatment plans as clinically indicated Educate patients and caregivers on: Home exercise programs Safety techniques Disease processes and functional limitations Complete timely, accurate, and compliant documentation per: Medicare Conditions of Participation Texas HHSC standards Agency policy Participate in interdisciplinary team communication and case conferences Coordinate care with physicians, nursing staff, and other disciplines Supervise Physical Therapy Assistants (PTAs), if applicable, in accordance with Texas regulations Adhere to infection control, patient confidentiality (HIPAA), and safety standards Perform discharge planning when therapy goals are met or services are no longer medically necessary Qualifications Required: Current Texas Physical Therapist license in good standing Graduate of an accredited Physical Therapy program Minimum 1 year of experience preferred (home health experience strongly preferred) Knowledge of Medicare home health regulations Valid Texas driver’s license, reliable transportation, and auto insurance Current CPR/BLS certification Ability to work independently and manage a mobile caseload Preferred: Experience with OASIS documentation Familiarity with EMR systems used in home health Strong patient education and communication skills Physical & Mental Requirements Ability to travel to patient homes within assigned service area Ability to lift, push, pull, and assist patients as needed Ability to stand, walk, bend, and perform therapeutic activities Ability to make independent clinical decisions within scope of practice What We Offer Competitive per-visit or salaried compensation (based on experience and role) Flexible scheduling Supportive leadership with clinical understanding Opportunity to work with a growing, mission-driven agency Professional autonomy with strong administrative support Equal Opportunity Statement Applesoft Homecare Services LLC is an equal opportunity employer.
