As a Hospice RN Case Manager, you’ll be a trusted clinical presence for your patients and their families. You’ll coordinate care, manage symptoms, provide education and support, and collaborate with physicians, caregivers, and the hospice interdisciplinary team.

• Manage and coordinate individualized hospice patient care plans

• Perform comprehensive nursing assessments and ongoing patient evaluations

• Provide expert symptom management and compassionate nursing care

• Educate patients and families about disease progression, medications, and comfort-focused care

• Collaborate with physicians, caregivers, and interdisciplinary team members

• Maintain accurate, timely, and thorough clinical documentation

• Provide emotional support and guidance to patients and loved ones

• Advocate for patient comfort, dignity, and quality of life

We’re seeking an RN who combines strong clinical skills with compassion, professionalism, and a genuine commitment to patient-centered care.

• Active Pennsylvania RN license

• Strong clinical assessment and organizational skills

• Excellent communication and interpersonal abilities

• Compassionate, patient-centered approach

• Ability to work independently and as part of a team

• Valid driver’s license and reliable transportation
