About the Role:
As a Registered Dietician (RD) at AdviniaCare Newport, you will be responsible for analyzing patients' health status, dietary restrictions, and goals to develop customized nutrition plans. Your main duties will include:
- Obtaining and evaluating nutrition histories of patients, assessing their knowledge of diet and kidney disease, and providing education to patients and their families
- Completing written monthly nutritional assessments and comprehensive patient assessments for patient care plans
- Participating in weekly care plan meetings and quality improvement team meetings
- Monitoring patient adherence and response to nutrition therapy, adjusting therapy as needed
- Documenting patient response to therapy in monthly progress notes
- Collaborating with a multidisciplinary team on patient care and management
Requirements:
• State and Federal dietitian certifications
• 3 years of SNF experience
