Physical Therapist

Role Overview: A licensed Physical Therapist providing personalized, in-home and senior living care to older adults. The role involves evaluating patients, developing individualized treatment plans, and delivering evidence-based therapy to enhance mobility, independence, and overall quality of life. Responsibilities Include Offering one-on-one therapy sessions in private residences and senior living communities Performing evaluations, creating care plans, and delivering evidence-based treatments Supporting patient wellness, longevity, and functional independence Documenting care accurately within EMR systems Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists Educating patients, caregivers, and staff on safety and aging-in-place strategies Utilizing telehealth/virtual care to extend access and flexibility Qualifications & Skills Valid Physical Therapy license and degree from an accredited program Proficiency with basic computer and EMR tools Current CPR certification Ability to meet physical demands such as lifting, repositioning, and assisting patients Key Attributes: Autonomy, flexibility, strong communication skills, and a passion for improving patient outcomes in community settings.
