Primary Care Physician

ChenMed is a physician‑led, mission‑driven primary care organization focused on Medicare Advantage seniors. Primary Care Physician (PCP) – Key Responsibilities Accountability for outcomes: Provide primarily patient care for a panel of up to 450 seniors, ensuring cost‑effective, high‑quality outcomes. Coaching for health: Act as a health coach, helping patients set short‑ and long‑term goals and tracking progress. Simplifying for action: Prioritize and simplify care plans to make behavior change actionable for patients and the care team. Conduct assessment, diagnosis, treatment, education, health promotion, care coordination and documentation for acute and chronic adult patients. Engage with hospitalists and, when needed, coordinate with transitional care teams (hospital, SNF, LTC, homebound). Lead a care team that includes a care promoter (medical assistant), a care facilitator, and a care coordinator. Maintain active center management, assist with recruitment of patients and staff, and cover for absent providers. Supervise, collaborate with, and remain accountable for an Advanced Practice Practitioner (APP) when in place. Perform additional duties as directed by management. Performance Metrics For each PCP’s patient panel, metrics such as patient admissions per thousand, appointment slot usage (18‑21 slots per day), CGCAHPS scores, clinical gap closures, and medical cost measures are regularly monitored. PCPs set goals for these metrics and work with center and market leadership toward those targets. Knowledge, Skills & Abilities Availability and accessibility to build trust; responsive to walk‑in visits and phone calls. Strong service orientation; treat patients as one would a family member. Evidence‑based medicine; stay current with clinical, nutritional, and lifestyle interventions. Develop continuous physician leadership skills. Commitment to high‑quality care and systematic improvement. Competent, compassionate influence of patients and teams. Self‑care to maintain personal health and wellbeing. Proficient in Microsoft Office Suite and related software. Willingness to travel locally, regionally and nationally for training and recruiting. Fluency in English, both verbal and written; additional languages may be required. Independent judgment in clinical decision‑making. Compensation Pay Range: $221,141 – $315,915 (annual). Final compensation depends on experience, education, location and other factors. Bonus or commission opportunities may be available.
