Child & Youth Services Case Manager
Centre County Government
Centre County Government is seeking a dedicated human services professional to support a caseload of clients and families in our community. You will conduct intake, perform interviews, develop and monitor case plans, and coordinate with community services to meet clients' needs. The role requires documentation, on-call duties, and occasional home visits or transportation arrangements to appointments.
