Social Services Coordinator

Winston Manor – Chicago, IL

Winston Manor is seeking a compassionate, organized, and dependable Social Services Coordinator to join our team. The Social Services Coordinator supports residents by addressing psychosocial needs, coordinating services and resources, and working collaboratively with families and the interdisciplinary team to promote resident well-being and quality of life.

This position is ideal for someone who is compassionate, communicative, organized, and committed to advocating for residents in an Intermediate Care Facility environment.

Responsibilities

• Identify and assist with residents' psychosocial and social service needs

• Develop positive and professional relationships with residents and families

• Participate in resident assessments, care planning, and interdisciplinary care conferences

• Assist with developing and updating social service components of resident care plans

• Provide residents and families with information, support, and appropriate community resources

• Assist residents with adjustment to the facility and changes in their care needs

• Support the admission, transfer, and discharge planning processes as applicable

• Advocate for resident rights, dignity, preferences, and quality of life

• Address resident and family concerns and communicate issues to the appropriate department or facility leadership

• Coordinate with nursing, behavioral health, activities, dietary, and other members of the interdisciplinary team

• Maintain accurate and timely social services documentation

• Assist with obtaining necessary resident information, documentation, and resources

• Participate in facility meetings, quality improvement initiatives, and regulatory surveys as needed

• Maintain resident privacy and confidentiality

• Follow facility policies, procedures, and applicable regulatory requirements

• Perform additional Social Services responsibilities as assigned

Qualifications

• Education and experience appropriate for Social Services responsibilities in a long-term care or ICF setting

• Previous experience in social services, behavioral health, healthcare, long-term care, or a related environment preferred

• Strong communication and interpersonal skills

• Strong organization, documentation, and time-management skills

• Ability to communicate professionally with residents, families, staff, and outside providers

• Ability to work effectively as part of an interdisciplinary team

• Compassionate, dependable, professional, and resident-focused

• Must meet applicable health and background requirements

Schedule

• Full-time opportunity

• Schedule may vary based on resident and facility needs

• Occasional weekend or holiday availability may be required
