Responsibilities Coordinate and conduct initial and comprehensive MDS assessments for residents, ensuring timely and accurate submissions to CMS Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team Identify and resolve discrepancies in MDS data, documentation, and care planning Collaborate with healthcare teams to identify and address resident needs, providing education and guidance as needed Maintain accurate and up-to-date records, including MDS assessments, care plans, and resident data Ensure compliance with federal, state, and local regulations, as well as organization policies and procedures Participate in quality improvement initiatives and contribute to continuous quality improvement efforts Requirements 2 years of experience preferred High school diploma or equivalent required; degree in a related field (e.g., healthcare administration, nursing) preferred What We Offer PLEASE LIST BENEFITS #J-18808-Ljbffr
