Schedule Flexible (3, 4, or 5‑day work week) Full Time (30–40 hours weekly) Day shift only No weekends Fixed schedule/no on‑call Key Responsibilities Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed) Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists Collects and records relevant patient health data in the course of/pre‑post hemodialysis treatment Develops and maintains a plan of care that describes interventions to attain expected outcomes Implements and records interventions to attain expected outcomes in EMR Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.) Maintains a positive and respectful attitude to all patients and colleagues Occasional travel to other nearby locations Medication administration and documentation Requirements BS/AA/diploma in nursing with at least 12 months of experience (3 months in hemodialysis) preferred Valid RN license BLS certification Ability to lift up to 35 pounds is required Benefits Depending on your job status, you may be eligible for the following benefits: Medical, Dental, Vision, & Life Insurance 401(k) package Flexible Spending Accounts Education Assistance Certification & Professional Membership Reimbursement Compensation offers are based on education, experience, skills, and geographic region. #J-18808-Ljbffr
