divh2Caregiver Position/h2pCaregivers assist agency clients with tasks of daily living as outlined in a written care plan established by the agency Care Manager. This may include the provision of hands-on personal care services as well as tasks generally considered to be companion or homemaker in nature, such as light housekeeping, meal preparation, shopping, errands, transportation and companionship./ppMinimum qualifications:/pulli1,000 hours of long-term care with geriatric or chronically ill persons. This translates to approximately 6 months of full-time work or 12 months of part time work. Exceptions to this requirement are for recently licensed Certified Nursing Assistants (CNAs) or Home Health Aides (HHAs), or persons who will only be providing companionship and housekeeping services./liliCNA or HHA license preferred./liliProof of current TB testing./liliMust be physically able to perform the duties of this position./liliMust have available reliable transportation; if transporting clients, must also have proof of a valid drivers license and automobile insurance./liliMust possess excellent communication skills./li/ululliBe mature, caring, punctual, and reliable./li/ulliPass a criminal background check (national and any required by state)./liliProvide verifiable professional and personal references./lipEssential duties and responsibilities:/pulliUnderstand a clients care plan and adequately implement it./liliAssist clients according to their care plan which may include: companionship, meal preparation and service, light housekeeping, transportation, shopping, medication reminders, bathing, body care, dressing, feeding, ambulation, positioning, transferring, toileting, and protective supervision./liliNotify the Care Manager of client concerns, changes in care or client condition and client emergencies./liliNotify the Care Manager and/or Staffing Coordinator of any safety concerns./liliFollow company Infection Control practices; notify the office if in need of appropriate Personal Protective Equipment (PPE) or supplies./liliFollow the ClearCare Telephony Instructions to clock in and clock out for all scheduled shifts and accurately update your task status for each client prior to clocking out and leaving the clients home. For compensable worked hours that are not allocated to a specific client (such as training time), you must use and submit the Time Log and Time Certification for Non-Visit Time form for each period pay period./liliConform to the company dress code as specified in the Employee Handbook./liliReport to work on-time and ready for the day./liliComplete ongoing training as required by client care needs and/or changes in company policies and procedures./liliAttend staff meetings./liliAdhere to all company policies and procedures as detailed in the Employee Handbook./li/ulpOrganizational relationship: The caregiver is supervised by the Care Manager and receives scheduling direction from the Staffing Coordinator./ppPhysical and environmental demands: Reposition and transfer clients, sustain weight of clients using proper transfer safety techniques and equipment (example: use of transfer belt while assisting client to transfer from bed to chair), and travel to clients residence place of residence using a reliable means of transportation./p/div
