In-Home Health Evaluation NP Contractor (Flexible)

Signify Health, part of CVS Health®, seeks board-certified nurse practitioner contractors to conduct in-home Health Evaluations for Medicare Advantage, Medicaid, and some commercial plan members. You’ll visit patients in their homes, review medications and history, and complete a health assessment using a pre-populated form. This role is independent contractor with a flexible schedule and visits on Mon-Sun; no overnight shifts. #J-18808-Ljbffr Signify Health
