Registered Nurse Case Manager

The Registered Nurse Case Manager coordinates the management of care, ensuring optimum utilization of resources, service delivery, and compliance with external review agencies. This role provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. The RN enhances the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness through the integration of case management, utilization review and management, and discharge planning functions. Acting as a patient advocate, the RN addresses the holistic needs of the patient through effective resource utilization and discharge planning.

• Coordinate care management and ensure optimal resource utilization.
• Conduct comprehensive assessments and develop care plans.
• Implement care plans and evaluate individual patient needs continuously.
• Collaborate with interdisciplinary teams (IDT) for effective patient care.
• Advocate for patients, addressing their holistic needs and preferences.

This position is full-time, Monday through Friday, from 8:30 AM to 5:00 PM.

• Minimum of 2 years of recent RN Case Management experience.
• Experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
• Familiarity with MCG and discharge planning across acute care settings.
• Current California RN license.
• BLS certification through the American Heart Association.
