Home Health RN – Admissions & Case Management | $90K–$110K + Bonus Control

What You'll Earn:

• $90,000–$110,000/year for full-time roles, based on experience
• Top pay for RNs with OASIS and home health expertise

What You'll Do:

• Perform Start of Care, Recertification, and Discharge OASIS assessments
• Manage a home health caseload with clinical autonomy
• Collaborate closely with our supportive, nurse-led leadership team
• Strong clinical assessment and documentation skills

Qualifications

What We're Looking For:

• Active RN license in Colorado
• Home health and OASIS experience preferred — the more the better
• Reliable, organized, clinically sound, and patient-centered
