Part Time RN Care Manager-Weekend

Role Care Manager responsible for facilitating discharge planning from admission to discharge, collaborating with healthcare teams, and managing complex psychosocial needs. Responsibilities Oversee transition planning, promote seamless care integration, coordinate patient throughput, ensure quality patient care and safety, and optimize performance using best practices. Qualifications Graduate of an accredited Nursing School, current license in the state, and a Bachelor's Degree in Nursing. Experience & Certifications 2-3 years in acute care preferred; care or utilization management experience desirable. BLS certification required; case management certification preferred. Skills & Knowledge Excellent communication, assessment, time management, critical thinking, advanced computer skills, and ability to interact effectively across organizational levels. Knowledge of care guidelines is a plus.
