Licensed Vocational Nurse (LVN) - Home Health

Assignment Duration

This is a 13-week contract position.

Schedule

AM Shift: Monday to Friday, 5 shifts per week, 8 hours per shift.

Job Description

We are seeking a qualified Licensed Vocational Nurse (LVN) to join our home health team. The successful candidate will provide routine visits and care for patients in specified coverage areas, while ensuring compliance with service area protocols.

Responsibilities

• Conduct routine patient visits and provide home health care services.
• Document patient progress and maintain updated records.
• Coordinate with healthcare teams and assist in care planning.
• Ensure compliance with healthcare standards and policies.

Required Experience / Certifications / Licensure

• Active LVN license.
• Recent home health experience is required.
• HomeCare HomeBase (HCHB) experience preferred; consideration may be given for those willing to learn.
• Ability to achieve 32 patient points per week.
