Registered Nurse – Home Health

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hours: 40 hours guaranteed
On-Call: Saturday/Sunday SOC/ROCs only
Productivity: 30 points/week (6 points/day)
Mileage: $0.49/mile

Service Area: Florence, Darlington, Williamsburg, Marion, Dillon & Marlboro Counties, SC

Position Overview

We are seeking an experienced Registered Nurse (RN) to provide comprehensive home health care and coordinate individualized treatment plans for patients in the community.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care, including wound care and IV therapy.
• Coordinate care with physicians, social workers, discharge planners, families, and other healthcare professionals.
• Manage care transitions and identify appropriate levels of care and support services.
• Educate patients and families on treatment plans and disease management.
• Maintain accurate clinical documentation and monitor patient outcomes.
• Identify opportunities for health promotion, illness prevention, and improved patient outcomes.
• Advocate for patient privacy, safety, and quality of care.

Requirements

• Current, unrestricted South Carolina RN License ; compact licenses accepted.
• Home Health experience required.
• HCHB and OASIS experience required.
• IV experience required.
• Associate's or Bachelor's Degree in Nursing.
• Strong clinical judgment and care coordination skills.
• Case Management certification preferred.
