As a clinical nurse, this individual spends majority of his/her time coordinating, ensuring, and providing direct patient care activities on the unit through: provision of nursing rounds, assessments, and treatments; facilitating admission, transfer, and discharge activities; provision of patient and family-centered education; care coordination and individualized treatment planning as applicable, etc.Job Responsibilities:-Performs and documents a comprehensive assessment in accordance with RNs scope of practice and hospital policies/procedures. -Considers the patient s culture, age and developmental level. -Coordinates initiation of patient database. -Identifies patient risk factors and presenting complaint. -Obtains patient health information from family or others as necessary. -Collects objective/quantitative and subjective/qualitative data. -Verifies initial admission assessment, if obtained by other members of the healthcare team.-Develops a problem list, prioritizing patient needs based on input from the inter-professional team as appropriate. -Design actions and interventions relevant to patient s culture and age. -Determines desired outcomes in coordination with patient and family in accordance with medical plan of care.-Fully implements patient care therapies and procedures with empowerment and clinical autonomy within scope of practice in accordance with established policies, procedures, professional guidelines, culture, and age of patient served. -Explains procedures, treatments and processes to patient and family. -Initiates action to alleviate patient s stress and discomfort. -Recognizes, responds to, and communicates changes in patient condition appropriately. -Conducts and documents ongoing comprehensive patient assessment and response to procedures and therapies using nursing process according to hospital policy, established standards, culture and age of patient served.-Revises and coordinates the plan of care and clinical pathway based on patient and family response and outcomes.-Mentors other staff members and nursing students. -Facilitates orientation of new staff. -Acts as a role model and resource to others in clinical role. -Demonstrates support of unit/house-wide professional governance/staff empowerment activities. -Demonstrates active participation in unit-based activities in support of organizational values/priorities. -Promotes a positive group dynamic and healing environment through words and actions.-Assures nursing excellence in accordance with accepted standards, evidence -based practice, current research, and caring philosophy. -Actively participates in department and/or unit quality improvement activities and core measures such as, but not limited to, patient satisfaction, patient falls, pressure ulcers, CAUTIs, auditing and monitoring of documentation. -Uses a holistic perspective regarding patient care. -Demonstrates critical thinking and creativity in the handling of patient issues.
