Who We're Seeking: We're seeking a Licensed Practical Nurse (LPN) to join our Home Health teams. Your role involves monitoring and reporting on patients' medical and emotional well-being. Additionally, you'll communicate with the overseeing RN to coordinate support from the home health team. This position focuses on delivering care to individuals in their homes. Your Responsibilities: Determine patient care needs and develop goal-oriented care plans in consultation management. Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations. Work autonomously under RN direction and utilize supervisory support when necessary. Empower clients to explore care options and alternative plans to achieve desired outcomes. Participate in on-call rotation and respond to calls promptly.
