Community Case Manager & Care Coordinator

Bay Area Community Services (BACS) is looking for a Care Coordinator in Sacramento to provide high-quality therapeutic interventions and case management services to individuals experiencing chronic homelessness. This community-based role requires direct, face-to-face service throughout Sacramento County. The ideal candidate will have at least 4 years of experience in social services, specifically with the homeless population, along with a valid CA driver's license.
