Hospice RN Case Manager

Join Addus Family company as a Hospice RN Case Manager and provide compassionate, in-home end-of-life care. You’ll manage a caseload of hospice patients, conduct comprehensive assessments, and develop individualized care plans that support comfort, dignity, and family needs. Collaborate closely with social workers, aides, chaplains, and physicians to coordinate services, manage pain and symptoms, and ensure high-quality, compliant care.
