About the Position

Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Duration: 13 Weeks Shift: Day Shift Hours per Shift: 8 hours (Monday - Friday) Experience: Minimum of 3 years Hospice experience (home or inpatient) License: BLS, Driver's License, personal vehicle, car insurance required Certifications: None specified Must-Have: - Minimum of 3 years hospice experience (home or inpatient) - BLS certification - Driver's License - Personal vehicle and car insurance - HCHB EMR experience Description: Provide hospice homecare to patients. Care will be focused in specific areas. Current department census ranges from 120-130 patients. Average caseloads consist of 12-15 patients, typically with 5 visits per day. Responsibilities include managing end-of-life care for adults, primarily geriatrics. Common patient diagnoses include CHF, COPD, respiratory failure, kidney failure, liver failure, and neurological conditions such as Alzheimer’s and dementia. Position involves managing oncology patients as well. Critical thinking and strong assessment skills are essential for this role. Independence and autonomy are required when providing care. General wound care, Pleurx drains, and the management of opioid/narcotic medication administration education are included in the role. All aspects of the hospice experience will be managed. Several standing orders limit the need to contact a physician for every order. Support is available from Home Care aids who work independently. Triage RN assistance is provided for unexpected needs and patient emergencies. Charge nurse support is available via phone. Administrative on-call support is available daily. Floating may occur to patient homes within assigned territory. Possibility to assist other RNs if caseload is low. Weekend rotation options are available if desired. No mandatory on-call duties required. Holidays require coverage for 4 hours, with the possibility of up to 2 holidays per contract based on the time of year. Requests for time off are approved through management. Scheduled hours are Monday to Friday, from 08:30 to 17:00. Set scheduling is in place. Unit culture promotes excellent teamwork and cohesiveness. Scrub color is navy.
