Primary Care Physician

We are seeking a compassionate and dedicated Primary Care Physician to deliver high-quality, value-based care to an established panel of predominantly geriatric patients. This role emphasizes preventive medicine, chronic disease management, and building meaningful, long-term patient relationships in a supportive outpatient environment. You will work within a collaborative, multidisciplinary care model designed to improve patient outcomes, enhance access to care, and reduce unnecessary hospitalizations. Practice Info 100% Outpatient (Geriatrics-focused position) Collaborative, multidisciplinary care model Physician-led, team-oriented environment focused on quality over volume Strong emphasis on preventive care and population health management Robust clinical support allowing you to focus on patient care Responsibilities Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions Deliver preventive care, wellness visits, and health screenings with a focus on early intervention Develop and manage individualized care plans for a predominantly geriatric population Collaborate closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care Utilize electronic health records (EHR) to document clinical encounters and track patient outcomes Engage in quality improvement initiatives and contribute to a culture focused on continuous care enhancement Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices Compensation Competitive base salary production-based bonus Sign-on bonus and relocation assistance (based on qualifications) Salary: $280,000 Benefits Fully funded malpractice insurance CME allowance and licensing renewal support 401(k) with employer match Comprehensive clinical and administrative support Shift & Schedule Monday–Friday, 8:00 AM – 5:00 PM No weekends or hospital rounds Requirements MD or DO from an accredited medical school Internal Medicine or Family Medicine or Geriatric Medicine Active, OK license Current DEA registration At least 3 years of experience in Outpatient Primary Care Strong clinical judgment with a focus on preventive and longitudinal care Excellent interpersonal and communication skills Commitment to a team-based, patient-first care modelaa415a4b-8b21-40fc-a65c-70d2b25ca29a
