Travel Physical Therapist (PT) – Home Health

Largo, Florida

We are seeking a Travel Physical Therapist for a Home Health contract in Largo, Florida . This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to adult and geriatric patients in their homes.

This is a great fit for a PT who enjoys autonomy, patient-centered care, and helping individuals improve mobility, strength, safety, and overall independence in their home environment.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on physician orders and patient goals.
• Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
• Provide gait training, balance training, transfer training, therapeutic exercise, fall prevention, and functional mobility interventions.
• Educate patients and caregivers on home exercise programs, safety strategies, and proper equipment use.
• Complete timely documentation and maintain home health compliance standards.
• Manage a daily home health caseload, with skilled visits expected in the field.
• Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
