As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. In this role, you will work onsite at our Jourdanton clinic. You will report to our Associate Medical Director. Team-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness. More Time with Patients: See fewer patients per day and spend more time delivering meaningful care. We foster a welcoming, inclusive environment where teamwork and growth are prioritized. You will provide comprehensive care to seniors following clinical standards. You will collaborate daily with your care team to ensure coordinated, high-quality care. You will manage referrals, hospital/SNF coordination, DME, and home health services. You will document care accurately and efficiently with support from quality-based coders. You will participate in on-call rotation and support growth plans at your center. You will have passion for providing value-based care. You will have a Master's Degree in Nursing or completion of a PA program with board certification. You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings. The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. 123,800 - $170,300 per yearThis job is eligible for a bonus incentive plan.
