Seven Healthcare are seeking an experienced Physical Therapist for a travel contract in Milledgeville, GA. This role involves conducting physical therapy evaluations and treatments based on physician orders, contributing to a team focused on patient rehabilitation and recovery. Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
• Complete comprehensive evaluations using appropriate assessment tools and procedures
• Review patient records for diagnosis, history, precautions, and contraindications
• Establish measurable goals and develop treatment plans for independence and function
• Regularly reassess treatment effectiveness and modify goals or treatment as needed
• Communicate patient needs and progress to the treatment team, physician, and relevant parties
• Consult with medical providers regarding patient evaluation, treatment, and discharge
• Recommend and assist in securing special adaptive equipment to improve function
• Maintain and submit required documentation per company, facility, and regulatory standards
• Supervise Physical Therapist Assistants and Aides in accordance with state practice act
• Attend staff development programs and maintain clinical practice skills

Requirements of the Physical Therapist

• Valid GA License (accepts Compact License)
• Experience with HCHB and previous home health experience
