Medical Case Manager- Primary Care (New Position)

Medical Case Manager – Primary Care Founded in 1987, Damien Center is Indiana’s oldest and largest AIDS service organization (ASO) and serves more than 8,000 individuals living with or at risk for HIV through a comprehensive, innovative approach to care and prevention. Our purpose is to be a trusted partner in providing services, education and advocacy for all people living with or at risk for HIV and any person seeking a safe and welcoming home for care. Our services include care coordination, clinical and pharmacy services, mental health, housing, and nutrition. Position Summary The Medical Case Manager is an essential part of the primary care team, focusing on coordinating care, managing chronic conditions, and ensuring that patients receive comprehensive and continuous care. This role involves developing individualized care plans, facilitating communication among healthcare providers, and providing targeted support to improve patient outcomes. Duties and Responsibilities Conduct periodic biopsychosocial assessments to ascertain the level of care required. Develop and implement individualized care plans in collaboration with healthcare providers. Make appropriate referrals to community resources to meet patient needs. Assist clients with enrolling in health insurance. Connect client to financial assistance programs to cover co‑pays, co‑insurance, and deductibles. Facilitate communication between patients, primary care providers, specialists, and other healthcare services. Monitor patient progress and adjust care plans as needed. Provide education and support to patients with chronic medical conditions. Monitor adherence to treatment plans and medication regimens. Assist patients with navigating the stages of change to improve health outcomes. Conduct regular follow‑up appointments to assess and address patient needs. Build strong, trusting relationships with patients to encourage active participation in their care. Conduct patient education sessions on disease management, medication adherence, and healthy lifestyle choices. Address barriers to care, such as the social determinants of health. Engage clients in meaningful risk reduction conversations regarding HIV treatment and prevention strategies such as U=U, PrEP, condoms, and testing. Encourage healthy lifestyle choices through patient education. Track and follow up on preventive care measures to ensure compliance. Complete necessary paperwork and data entry, including entering case notes, within 24 hours of client interaction. Perform special assignments and other work, on an as‑needed basis. Education and/or Experience Bachelor’s degree in Social Work or related field. Minimum of 2 years of experience in case management, preferably in a primary care or chronic disease management setting. Strong communication and interpersonal skills, knowledge of chronic disease management, ability to work collaboratively with healthcare providers, and proficiency in patient education. Experience working with diverse populations and understanding cultural competency. Experience working with adolescents 16 – 17 years and their families, especially in healthcare settings, is highly valued. Bilingual in Spanish preferred but not required. Knowledge, Skills, and Abilities Possess superior organizational and administrative skills. Excellent interpersonal and communication abilities. Excellent computer skills in a Microsoft Windows environment. Ability to work independently and achieve high standards of productivity, meeting deadlines and delivering results. Strong calendaring, organizational, and document management skills. Proactive problem‑solving and communication with leadership. Knowledge of office management systems, procedures, and office equipment. Excellent time‑management skills and ability to prioritize work. Familiarity with electronic health record (EHR) systems. Ability to conduct health assessments using standardized tools. Qualifications To perform this job successfully, an individual must be able to perform each essential job function satisfactorily. Physical Demands Regularly required to sit, reach with hands and arms, talk, and hear; frequently use hands to touch and handle objects; often stand or walk. Occasionally lift and/or move up to 10 pounds. Required vision abilities include close vision and the ability to adjust focus. Work Environment Noise level is minimal to moderate. Individuals should maintain personal and professional competency and work to create an environment of courtesy, respect, inclusion, and positivity in all interactions both internal and external to the organization. Vaccination & Health Requirements All team members must be vaccinated against influenza and receive a tuberculosis skin test (or chest x‑ray) annually, barring an approved religious or medical exemption. FLSA Status Hourly, Full‑time, Non‑Exempt. Leader Linkage Services Senior Program Manager. Salary $26 per hour. Benefits 150 hours of paid time off in the first year, followed by 195 hours per year moving forward. 12 paid holidays. Medical coverage options include a PPO plan or a high deductible health plan.
