Wellness Home Services Licensed Practical Nurse (LPN) position In this role, you will be responsible for delivering individualized, one‑on‑one care tailored to each resident’s unique needs, under the direction of a Registered Nurse (RN) where required by state guidelines. Specialize in managing chronic conditions, supporting post‑operative recovery, and addressing complex medical needs with a steadfast commitment to safety, dignity, and comfort. Perform medication management, vital sign monitoring, and collaborate with multidisciplinary care teams, while providing clear education and guidance to residents and families. Commit to delivering exceptional, high‑quality care that improves health outcomes, promotes independence, and enhances overall quality of life.
