Luminis Health Title: Treatment Assistant/Case Manager (TA/CM) Job Code: 001259 FLSA Status: Nonexempt Position Objective This position is responsible for general supervision of residents and their activities, milieu management, and facilitation of group meetings. Additionally, the role may serve as an assistant to the Adventure Therapist or as a primary counselor for a limited number of Pathways inpatients. As an assistant to the Adventure Therapist, the employee will receive training and certification. As a primary counselor, the employee will provide assessment, case management, counseling, collaborative treatment planning, and discharge planning to adults with primary substance use and co-occurring mental health disorders. The treatment philosophy is evidence‑based and focuses on multiple pathways to recovery, fostering patient success through individualized clinical interventions. Essential Job Duties Provides supervision and support for patients and their activities. Provides supervision of the resident environment and activities, clarifying program guidelines and rules. May conduct searches and drug tests according to program standards as needed. Orients new patients on treatment expectations, rights, and responsibilities. Transports patients to and from groups, breaks, meals, and outside appointments as needed. Under supervision, provides inpatients assigned with assessment, treatment planning, case management, counseling, discharge planning, and linkage to the appropriate level of care and/or community resources. Conducts individual counseling sessions by level of care, establishing a therapeutic alliance and maintaining effective working relationships with patients. Identifies patient strengths, integrates patient‑expressed goals and measurement‑based care into treatment planning with specified target dates, and documents progress. Facilitates psychoeducational and/or therapeutic groups as assigned and may cover groups due to a vacancy or absence. Demonstrates knowledge of ASAM, DSM‑V criteria, evidence‑based practices, and medications used in substance use treatment. Maintains chart documentation under supervision in accordance with Joint Commission, COMAR, Luminis Health, and Pathways’ requirements. Collaborates with other Pathways Counselors to manage emergent and complex patient problems, concerns, and other issues, including cross‑coverage and cognitive‑behavioral interventions as needed. Participates in clinical and multidisciplinary team meetings, supervision, in‑service, and other assigned training. Works collaboratively with patients and multidisciplinary teams, under supervision, to provide care tailored to each patient’s needs, strengths, values, and preferences. For TA/CM detailed to Adventure Therapy, assists Pathways Adventure Therapist in delivery of Adventure Therapy challenge course as assigned: Assists or delivers Adventure Therapy to patients along with management‑approved groups such as paramedics, police, and other public safety groups. Assists in maintenance and safety inspections of equipment and grounds. Maintains staff and course certifications. Staff may be asked to work in other areas of admission or treatment as needed. Educational/Experience Requirements Required Minimum Education: High school diploma or equivalent. An AA or BA/BS degree in a human services field is preferred. Required Minimum Experience: One year of work experience in behavioral health, individual and/or group counseling, and residential treatment experience preferred. Required License/Certifications Current approval as an Alcohol and Drug Trainee (ADT) from the Maryland Board of Professional Counselors and Therapists or Maryland certification as a Peer Recovery Coach is required. For TA/CM detailed to Adventure Therapy, Challenge Course Technology certification required within 6 months. Current Basic Life Support (BLS) certification through the approved Luminis Health system required within 30 days of hire. Driver’s license and driving record with no more than two points. Knowledge, Skills, Abilities Ability to work collaboratively with all types of disciplines and personalities while maintaining a positive and healthful work environment. Displays personal attributes of compassion, integrity, honesty, and a willingness to engage in critical self‑appraisal. Can balance initiative and independent work while accepting feedback and direction. Working Conditions, Equipment, Physical Demands Can exert up to 50 lbs of force occasionally, up to 30 lbs frequently, and up to 10 lbs constantly to move objects. Must be flexible to work occasionally outside normal daytime hours. Must observe and note patient behaviors as needed, including during group activities. Must hear typical environmental sounds, including some background noise. Can understand and operate from the conceptual basis of tasks and activities. Can move tables and chairs to set up rooms for counseling. Exposure to blood‑borne pathogens is reasonably expected.
