RN Patient Care Navigator - Stroke Program

Hourly Pay Range Hourly Pay Range: $40.45 - $62.70. The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights Position: RN Patient Care Navigator – Stroke Program Location: Evanston Hospital Per Diem (As Needed) Hours: Per Diem (as needed) for 12‑hour shifts 7:00 AM – 7:00 PM, must be flexible on days What you will do The Stroke navigator will render professional nursing care to assist neurologist, physicians, and APPs in coordinating the care of patients throughout the stroke continuum with primary focus on acute stroke alerts, supporting acute interventions, and collaborating with multidisciplinary team to ensure timely evidence‑based care. Ability to effectively function during emergent situations and manage multiple priorities. Employs critical thinking skills, confident neurological assessments, adheres to appropriate treatment protocols for specific emergencies, effectively communicates plan, assists with transport to emergency testing and transfer to higher level of care as indicated. Synergizes with stroke coordinator and neurosciences team to ensure care delivery meets comprehensive stroke center standards including helping maintain stroke program data. Provides stroke education to patients, families, and healthcare staff. Ultimately, this role serves as a clinical resource for stroke protocols, quality improvement initiatives, and regulatory compliance.
