The Community Health Worker position with CareSource’s Ohio Mobile program serves multiple Ohio communities including Toledo and surrounding areas. You will join an interdisciplinary care team to coordinate member care across hospital, home, and community settings.
Responsibilities include engaging with members, accompanying them to appointments, coordinating transport and services, verifying eligibility, helping develop personalized care plans, and empowering members to manage their health with
