Position Overview


We are seeking an energetic, proactive RN Clinical Coordinator to join our healthcare team in Sarasota, FL. This is a 100% office-based role that sits at the intersection of clinical care management, insurance operations, and team leadership.

In this role, you will act as the key operational bridge between adult patients, field nurses, collaborating providers, and insurance payers-ensuring seamless care plans, swift prior authorizations, and total regulatory compliance. You will also assess, diagnose, and manage treatment plans for assigned patients while providing clinical and administrative support to maintain operational excellence.

Note on Client Preference: This position requires full operational focus; candidates who currently own or operate their own private practice will not be considered.

Key Responsibilities

• Care Coordination & Clinical Oversight: Review shift notes, collect/track physician orders, assess patient conditions, and manage a daily caseload of 20-30 adult patients receiving long-term care (LTC) and home health services. Establish and diagnose individualized patient treatment plans, monitor changes in symptoms, and facilitate referrals to specialists or medical facilities.
• Insurance & Authorizations: Process insurance eligibility verifications, submit and track prior authorizations, and communicate directly with payers to ensure coverage validation, prevent gaps, and maintain uninterrupted patient care.
• Referral Management: Review and process electronic client referrals coming in from hospitals, rehabilitation centers, and nursing facilities.
• Interdisciplinary Liaison: Serve as the primary point of contact for collaborating physicians, clinical field staff, clients, families, and insurance representatives to maintain seamless communication and continuity of care.
• Compliance & Audit Readiness: Ensure clinical documentation and workflows align with Florida healthcare regulations, Medicaid LTC standards, state/federal compliance, and internal quality assurance audits.
• Patient Education & Support: Educate patients and families on health maintenance, disease prevention strategies, and long-term care transitions.

Qualifications & Requirements

• Licensure: Active Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) license in the state of Florida.
• Experience: Minimum 2+ years of recent experience in Clinical Coordination, Case Management, Long-Term Care (LTC), Home Health, Skilled Nursing, Rehabilitation, or Managed Care environments.
• Clinical Skills: Comfortable independently performing comprehensive physical and clinical head-to-toe patient assessments.
• Insurance Expertise: Direct, hands-on experience handling insurance eligibility verifications, prior authorizations, and payer communications required.
• Tech & Software Skills:
  
  
  • Hands-on experience using PDFfiller to complete and manage Plans of Care (POCs).
  • Proficiency in operating Apple products / Mac environment.
  • Familiarity with EMR systems, healthcare documentation platforms, and medical software.
• Regulatory & Quality Standards: Strong understanding of clinical documentation standards, Florida healthcare compliance, quality assurance, and audit readiness.
