Job Details Level: Experienced Job Location: Saratoga, CA 95070 Position Type: Full Time Salary Range: $55.00 - $65.00 Hourly Travel Percentage: None Job Shift: AM Job Category: Health Care FLSA Status: Non-Exempt Position Overview Provides and directs all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to organization’s guidelines, professional standards and community needs. Serves as a patient and rehabilitation advocate for current prospective patients and all internal and external customers. Participates in quality assurance and customer service activities. Reporting Responsible to the Facility Rehab Director. Essential Job Functions Initiate evaluations per M.D. orders, payer authorization, and facility/payer source guidelines. Develop and provide treatment consistent with benefit and reimbursement level for best functional outcome for patient. Educate staff, other health care professionals and third party payer on the benefits and functional outcomes of intervention. Complete documentation including evaluations, daily progress notes, summaries, or monthly reports as required. Key information into computer systems, including EMR and rehab systems. Screen all individuals who may benefit from therapeutic intervention. Use professional judgment to ensure safety of self, patients and others at all times. Achieve quality patient care through interdisciplinary co‑op, co‑treatment, setting up protocols and programs. Attend continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques. Provide in‑service training sessions or other educational presentations to company, team or facility to enhance clinical and professional knowledge and skills. Provide assistants and rehab techs clear directives concerning patients and treatment plans prior to initiation of patient care. Confere with Program Specialists and supervisor to develop and implement new programs/techniques to enhance program utilization. Maintain appropriate state licensure, certification and credentialing. Submit billing, payroll, and expense documentation in a timely manner. Make appropriate referrals to other members of the rehab team. Job Functions Direct and supervise the work of assistants, rehab aides and students as applicable with State and Federal guidelines. Follow all organizational and facility safety procedures regarding personal protection and patient safety. Key information into computer systems. Expand and enhance self, in both clinical and professional knowledge through associations and continuing education. Support and participate in department operations and development. Communicate patient status and needs to patients, families, caregivers, or other members involved with patient care. Physical Capacities Move intermittently throughout the workday. See and hear or use prosthetics that enable adequate function. Meet general health requirements including annual TB screening and physical examination. Lift up and over 100 pounds with assistance. Sit and walk for extended periods of time. Travel to assigned destinations in a reasonable timeframe. Bend, squat, push, stoop, and pull as needed, especially in resident care activities. Coping with the mental and emotional stress of the position. Environmental Conditions Work inside, normal temperatures, some noise, occasional fumes/odors, chemical exposure and potential exposure to bloodborne pathogens. The job requires coping with stresses associated with an intimate 24‑hour skilled care environment serving disabled and cognitively impaired residents in an aging population. These stresses include emergency health or safety response, weekend and holiday duty, unusual or impaired behavior by residents, family reactions, death and dying, oversight of state surveyors, ombudsmen and federal officials, presence of consultants and attorneys, and variable involvement of medical staff. Qualifications / Requirements Graduate of an approved curriculum in Occupational Therapy. In good standing with all regulatory agencies and licensing boards. Working knowledge of Medicare and other payer sources. Full knowledge of resident’s rights. Current, active license as Occupational Therapist, able to practice unencumbered. CPR certification required for license renewal. Experience in long‑term care preferred. Able to work on multiple tasks at the same time. Proficient in reading, analyzing and interpreting common scientific and technical information, and effective verbal communication in English. Mathematical skills: add, subtract, multiply and divide in all units of measure using whole numbers, common fractions, decimals, weight and volume measurements.
