Position Overview Elderwood at Williamsville is seeking a compassionate and dedicated Physical Therapist to join our skilled nursing facility team. Our Physical Therapy professionals play a vital role in ensuring the health, safety, and well-being of our residents by providing high-quality, individualized therapy services that support recovery, mobility, and independence.

Responsibilities

• Provide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting
• Develop and implement individualized treatment plans based on resident needs and physician recommendations
• Administer skilled physical therapy interventions to improve mobility, strength, balance, and overall function
• Monitor resident progress and adjust treatment plans as appropriate
• Collaborate with interdisciplinary team members to ensure coordinated and effective care
• Maintain accurate and timely clinical documentation in accordance with regulatory standards
• Ensure the highest degree of quality care and resident safety is maintained at all times

Qualifications

• Current State Physical Therapy License required
• Strong clinical assessment and treatment planning skills
• Ability to work collaboratively in a multidisciplinary team environment
• Commitment to providing compassionate, resident-centered care
