For more information, visit Job Title: Advanced Practice Provider (APP) – Outpatient Requisition Number: req29344 Location: Center for Behavioral Health Recovery Job Description Job Summary The Outpatient Behavioral Health Advanced Practice Provider is responsible for delivering comprehensive mental health care in an outpatient setting, including diagnosing and treating various psychological conditions. This role involves conducting assessments, developing and implementing individualized treatment plans. Essential Duties and Responsibilities Coordinate multidisciplinary care for all patients to ensure high‑quality, optimized outcomes. Conduct comprehensive psychiatric evaluations and mental‑health assessments to determine patient needs and treatment goals. Diagnose mental‑health conditions and develop individualized treatment plans, including medication management and other interventions. Provide evidence‑based, compassionate care to improve patient outcomes and enhance patient satisfaction. Educate patients and families about care plans, treatments, and health management. Make appropriate referrals to support comprehensive and continuous care. Collaboration & Communication Maintain a collaborative environment with providers, clinic staff, outpatient clinic leaders, and behavioral health physician and APP leaders. Communicate effectively across disciplines to support integrated care delivery. Quality Improvement & Compliance Participate actively in Performance Improvement Programs and Peer Review committee meetings. Monitor and ensure compliance with care management guidelines established by JPS Health Network, Acclaim and relevant accreditation agencies. Duties and Essential Job Functions Coordinate multidisciplinary care delivered to all patients to optimize care. Review patient medical charts to diagnose, treat, and provide care management and education to patients and families. Make appropriate referrals as necessary. Monitor guidelines for the management of care and ensure services comply with guidelines established by JPS Health Network and Acclaim Physician Group. Provide evidence‑based compassionate care to all patients to improve outcomes and increase satisfaction scores. Maintain a collaborative environment with referring physicians, residents, clinical staff, and support staff to provide quality patient care. Support academic, educational, and research endeavors. Attend Performance Improvement Programs and Peer Review committee meetings; participate actively. Partner and communicate with acumen to build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Physician Group. Serve as a resource to the community. Perform other job‑related duties as assigned. Qualifications Required Education and Experience Graduate of an accredited Physician Assistant program OR Nurse Practitioner program (PMHNP certification) Required Licensure/Certification/Specialized Training Current, unrestricted medical license to practice in the State of Texas.
