Core Responsibilities Conduct comprehensive initial and ongoing nursing assessments for patients across a defined service territory Develop, implement, monitor, and regularly update individualized patient care plans in collaboration with physicians and interdisciplinary team members Administer medications, wound care, injections, IV therapy, and other skilled nursing interventions as ordered Educate patients and their family members on disease management, medication adherence, safety practices, and self-care techniques Monitor patient progress closely and communicate changes in condition promptly to the supervising physician and care team Maintain accurate, timely, and compliant clinical documentation using electronic health record systems Coordinate seamlessly with physical therapists, occupational therapists, social workers, and other healthcare professionals to ensure comprehensive, continuous patient care Participate in quality improvement initiatives and maintain compliance with all relevant regulatory standards and agency policies Respond to urgent patient needs and provide telephone triage support when required

Must-Have Requirements Active Registered Nurse license in good standing Minimum of one year of clinical nursing experience, with home health, medical-surgical, or critical care experience strongly preferred Current Basic Life Support certification required Valid driver's license and reliable personal transportation for patient home visits Proficiency with electronic health record documentation systems Strong organizational, time management, and prioritization skills Excellent interpersonal communication and patient education abilities Ability to work both independently and collaboratively within a multidisciplinary team environment
