Contract | 13-Week Assignment | Skilled Nursing Facility (SNF)
Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to restore or improve physical function and mobility
- Develop and implement individualized physical therapy treatment plans based on patient assessments and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory requirements
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and family members on therapeutic exercises and home safety strategies
Required Qualifications
- Active Physical Therapist (PT) license in the state of South Carolina or compact eligibility
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a skilled nursing facility (SNF) or post-acute care environment
Preferred Experience / Skills
- Prior travel or contract therapy experience
- Familiarity with MDS documentation and Medicare guidelines in a SNF setting
