Bilingual RN Care Manager: Chronic Disease Navigator
San Bernardino County
Arrowhead Regional Medical Center (ARMC) seeks an RN Care Manager to work at the top of their scope with chronically ill patients and high hospital recidivism. The role focuses on educating patients about disease processes, recognition of warning signs, and proper home self-management to navigate the healthcare system effectively.
