Private Duty LPN

Role: Home and Community Based Services Licensed Practical Nurse (LPN) Responsibilities: Provide direct nursing care to acutely and chronically ill patients in their homes, document clinical notes, coordinate patient care activities, and ensure high-quality services within the scope of practice. Participate in care coordination by reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, and healthcare team members. Ensure compliance with infection control procedures and regulatory documentation standards. Qualifications & Skills: Current and unrestricted LPN licensure in the practice state Valid driver’s license, vehicle insurance, or access to reliable transportation Preferred: CPR certification, ability to work flexible hours, strong communication, organizational, and independent working skills Key Duties: Deliver clinical services per plan of care, document visits thoroughly, participate in care coordination, and function effectively in diverse home settings. #J-18808-Ljbffr LiveWell Homecare Agency, LLC
