Pharmacy Manager-Internal Medicine/Transitions of Care-Mount Sinai Brooklyn-FT-Day

Description Responsible for carrying out clinical pharmacy services, promoting clinically and evidence-based rational drug therapy, and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs. Serves as a clinical pharmacy resource for the health system and its affiliated pharmacy services. Responsible to serve as a preceptor for the residency program, and for students from local colleges of pharmacy. Conducts research and other scholarly activities throughout the year with the goal of publishing. Supervises and guides pharmacists on the optimal provision of care to ensure professional development, and through use of their independent judgment, directs and disciplines their employees. Other responsibilities include patient care rounds, clinical interventions, medication use evaluations and institutional committee membership. Responsibilities Participate in daily multidisciplinary internal medicine hospitalist rounds Provide accurate and timely medication information to healthcare staff and patients Provide clinical and operational pharmacy support to pharmacy and medicine teams Collaborate with the Director and Pharmacy and Associate Director of Pharmacy to develop and implement clinical pharmacy programs and services in support of the pharmacy department mission Serve as an active participant in local and system committees in related clinical practice areas as assigned Actively participate in the Antimicrobial Stewardship program efforts Contribute to the Transitions of Care services and programs Identify, design, and analyze drug utilization reviews to promote rational drug therapy and determine action plan based on results Participate in departmental and hospital quality and cost-savings initiatives Sustain the hospital formulary, minimizing non-formulary procurement, utilizing therapeutic interchanges and promote rational drug therapy selection Work collaboratively with other health care providers and the clinical team members to develop and maintain a sound formulary system to support evidence based pharmaceutical care and cost-effective drug utilization. Coordinate formulary reviews, drug use evaluations, and quality assurance programs. Serve as a preceptor for pharmacy students and PGY1 pharmacy residents Develop and implement systems and procedures to maintain an efficient and effective workflow within the pharmacy and/or assigned areas Provide direction and guidance for the safe processing and dispensing of all medications for assigned area Address issues or services requiring intradepartmental coordination proactively Ensure an adequate inventory of pharmacy products and supplies to meet department demands Ensure the department is in compliance with all federal, state and local regulations, accrediting agencies; and Mount Sinai Heath System policies and procedures. Contribute to managing various personnel functions such as hiring, training, performance appraisals, coaching, disciplinary actions, and terminations. Exchange information and provide input for inter-and-intra departmental meetings Serve on a rotating basis, as an on-call administrator and troubleshoot issues that arise evenings, weekends, and holidays. Maintain adequate documentation to satisfy JCAHO requirements or other regulatory agencies Perform other duties as assigned Qualifications Doctor of Pharmacy Degree required Current NYS Pharmacist license and registration ASHP-Accredited PGY-1 required, PGY-2 residency preferred Board certification (BCPS, BCCCP) strongly encouraged Hospital pharmacy and/or clinical experience; management experience preferred Employer Description Strength through Unity and Inclusion The Mount Sinai Health System is committed to fostering an environment where everyone can contribute to excellence. Compensation The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $127,044 - $190,565 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
