Registered Nurse - Home Health

Location: Rockville, MD

Assignment Duration: This is a 13-week contract position.

Schedule: Monday through Friday, 8:00 AM - 5:00 PM

Job Description: The Registered Nurse (RN) will provide home health care services, primarily focusing on adults and geriatric patients. The role includes managing a caseload involving discharges from hospitals, medical offices, and skilled nursing facilities.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor patients' health and report significant changes.
• Coordinate care with other healthcare professionals.
• Provide education and support to patients and families regarding health management.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health and HCHB experience required.
• Active RN license in Maryland or compact state required.
• BLS certification is required.
• Experience in case management and proficiency with OASIS documentation is preferred.
