Registered Nurse - Primary Care

Coastal Healthcare is seeking a Community Health Nurse to provide direct patient care and treatment in primary care clinics, VA, or School-based Health Centers (SBHC) throughout Lincoln County. The role focuses on promoting health through education, care coordination, and health maintenance. Compensation $41.56 – $55.85 per hour* *Wage determined in accordance with Oregon Pay Equity laws based on education and experience. Benefits Medical, Dental, & Vision Coverage 11% County contribution to 401(k) County funded Health Savings Account (HSA) $40,000 County Paid Life Insurance Employee Assistance Programs (EAP) Location Depoe Bay, Oregon Responsibilities Community Health Nurses follow six standards of practice: Assessment – Collect comprehensive data related to the health of the population served and document it in clear, accessible terms. Diagnosis and Priorities – Analyze assessment data to derive a nursing diagnosis and priorities, facilitating population involvement in care planning. Outcome identification – Identify culturally relevant, health‑literacy‑based, evidence‑ingrained outcomes and measurable objectives. Planning – Develop a client‑centered plan using best practices, identifying strategies and action steps in language understandable to the care team and population. Implementation – Oversee plan execution through treatment, case management, referrals, collaboration with community resources, and health promotion, documenting activities and coordination status. Evaluation – Apply ongoing assessment data to revise plans and interventions, documenting changes that enhance effectiveness. Additional responsibilities for Primary Care and School‑based Health Center CHN2: Apply theories and principles of primary care and adolescent health nursing practices in clinics and schools. Set priorities to provide quality, organized, and time‑efficient patient care; maintain confidentiality of patient records. Perform direct care: administer medications, observe and document patient responses or side effects, and use standing orders and protocols. Create a caring, compassionate environment that promotes respect for all patients and families and supports a high‑performance clinic culture. Coordinate with clinic and departmental personnel to resolve operational problems and provide comprehensive care. Meet training requirements per HRSA, HHS, and OHA guidelines. Participate in continuous quality improvement and analyze data system reports to guide practice improvement. Build professional rapport with clients, community, and health care partners to promote health. Other duties as assigned: Attend team meetings, trainings, and community activities to further the health of the community. Minimum Education Level Associate degree in Nursing from an accredited school of nursing. Minimum Experience Level Three years of nursing experience. Practicums and lived experience may substitute for years of experience. Driver’s License Oregon Driver’s License required within 30 days of hire. Other Licenses Must possess a valid Oregon Registered Professional Nurse license and maintain current licensure and certifications, providing verification at renewals. Desirable Qualifications Bachelor of Science degree in Nursing (BSN) with at least one year of primary care nursing experience. Knowledge Required Principles, practices, methods, and procedures of nursing applied to community and public health. Use and effects of medications and immunizations. Issues related to alcohol and drug abuse. Strength‑based model of behavior change. Problem‑solving and negotiation skills aligned with motivational interviewing and behavior change theory. Utilization of community resources. Medical record‑keeping practices. Functions and resources of private and public agencies.
