Productivity points standard: 30 points

Requirements
- Minimum of 1 year skilled home health physical therapy experience
- Oasis experience required
- BCLS certification required

Roles & Responsibilities
- Assess patient needs and develop individualized care plans
- Provide physical therapy treatments to clients in their homes
- Educate patients and families on exercises and therapy techniques
- Document patient progress and adjust care plans as necessary
- Collaborate with healthcare team members to optimize patient outcomes

Compliance / Notes
- Confirm reliable transportation
- Special requirements include primary COVID vaccine series plus one booster
