Position Summary:

The Social Work Consultant provides professional oversight, supervision, consultation, and guidance to the Social Services Department to ensure compliance with Maryland COMAR regulations, CMS Conditions of Participation, and facility policies. The consultant serves as a clinical resource to the Social Services Manager and interdisciplinary team while ensuring residents receive appropriate psychosocial services that promote their highest practicable physical, mental, emotional, and psychosocial well-being.

Qualifications:

• Current Maryland Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker-Clinical (LCSW-C).

• Minimum of three (3) years of long-term care or skilled nursing facility experience preferred.

• Knowledge of CMS regulations, Maryland COMAR requirements, resident rights, discharge planning, behavioral health, dementia care, and interdisciplinary care planning.

• Experience conducting clinical supervision and mentoring social services staff.

Essential Duties and Responsibilities:

Regulatory Oversight

• Provide professional supervision and consultation to the facility's Social Services Manager.

• Review social service documentation for accuracy, completeness, and timeliness.

• Monitor compliance with required psychosocial assessments, care planning, discharge planning, and resident rights.

Clinical Supervision

• Provide ongoing supervision and mentoring to the Social Services Manager.

• Meet with the Social Services Manager at least monthly (or as agreed by contract).

• Review complex resident cases and recommend appropriate interventions.

• Be available by telephone or electronic communication for consultation as needed.

Documentation

The consultant shall maintain documentation of:

• Dates and hours of consultation.

• Topics reviewed.

• Residents discussed.

• Recommendations provided.

• Follow-up actions.

• Staff education completed.

• Quality assurance activities performed.
