General Description:
The Case Manager plays a critical role in ensuring the physical, emotional, and developmental well-being of the youth served by Youth Emergency Services, Inc. This role goes beyond supervision-it requires proactive leadership, mentorship, and advocacy to close gaps in support and create transformational outcomes for clients. The Case Manager is responsible for providing a structured, safe, and empowering environment while fostering accountability, resilience, and success. Qualifications- Demonstrated leadership and problem-solving skills, with the ability to identify and close gaps in client care and support.
- Must be 21 years or older
- High school diploma or GED (Associate's or Bachelor's degree in social work, psychology, or a related field preferred).
- 1-2 years of experience working with youth, social services, or case management, with a strong ability to navigate challenges and drive meaningful outcomes.
- Proven ability to advocate for youth and families, ensuring that services, resources, and interventions align with client success goals.
- Strong decision-making and accountability mindset, with the ability to take initiative and proactively manage cases.
- Effective communicator and collaborator, skilled in coordinating multidisciplinary teams, engaging families, and building community partnerships.
- High emotional intelligence and resilience, capable of de-escalating conflicts, guiding youth through challenges, and modeling positive behaviors.
- Strong time management and organizational skills, with the ability to prioritize tasks in a fast-paced, dynamic environment.
- Tech-savvy and data-driven, with the ability to document progress effectively, analyze patterns, and use technology to enhance case management efficiency.
- Committed to continuous learning and professional development, with a growth mindset focused on improving self, team, and agency outcomes.
- Proactively manage client cases by identifying gaps in support, removing barriers, and driving positive change.
- Ensure timely and accurate documentation that reflects real-time client progress and intervention effectiveness.
- Serve as a client advocate in treatment plans, MDT meetings, court hearings, and interdisciplinary collaborations.
- Coordinate and oversee medical, educational, and therapeutic services, ensuring that every client receives the care they need.
- Bridge communication between therapists, schools, DFS, probation officers, judges, parents, and clients, ensuring seamless collaboration and accountability.
- Establish a culture of safety, stability, and structure that fosters client growth and responsibility.
- Ensure the home environment is clean, organized, and welcoming, reinforcing positive habits and respect for shared spaces.
- Prepare and provide nutritious meals that align with healthy lifestyle habits.
- Lead engaging group sessions, structured activities, and mentorship opportunities to support personal development.
- Provide active, engaged supervision, guiding clients with clear expectations, coaching, and consistent accountability.
- Actively contribute to a high-performing, mission-driven team, supporting colleagues and driving collective success.
- Take ownership of at least one agency committee, using insights and leadership to improve agency operations.
- Champion agency mission, policies, and values, ensuring daily actions reflect organizational goals.
- Model and reinforce positive behaviors and daily living expectations, creating a standard of excellence.
- Crisis Management & De-escalation: Ability to assess, intervene, and resolve potentially volatile situations with confidence.
- Strong Communication & Advocacy: Ability to connect, engage, and navigate conversations with youth, families, and professional partners.

- Understanding of Child & Adolescent Development: Ability to support youth growth with a trauma-informed, developmentally appropriate approach.
